In the state of Florida, a minor must have parental/guardian consent with a legal notarized form to obtain a piercing or tattoo.

This document is $10 for the notarization. There is a $30 travel and convenience fee during business hours 9am - 5pm OR a $50 travel and convenience fee for after hours from 5pm - 12pm midnight.

The purpose of an acknowledgement is for a signer, whose identity has been verified, to declare to a notary or notarial officer that he or she has willingly signed a document.

This document is $10 for the notarization. There is a $25 travel and convenience fee during business hours 9am - 5pm OR a $45 travel and convenience fee for after hours from 5pm - 10pm.

Oath, affirmation, jurat, notary, general notary work

Oath or Affirmation (Jurat)

Requires the signer to be compelled to truthfulness regarding the content of the document. The signer is required to take an oath/affirmation before the Notary in order for the receiving agency to trust the signer's statements within the document.
